To be considered for the Program, you must fill out this application honestly, accurately, completely, and to the best of your ability. Failure to do so may result in your elimination from consideration or from the show, as well as the forfeiture of any prizes, goods or services you may have received, whether or not you have already received them. All applicants must meet eligibility requirements, which includes being at least 18 years old (or the age of majority in your state of residence, if different), and being legally authorized to live, work and participate in the show in the U.S.
All applicants must review and submit a signed copy of the Eligibility Requirements & Release.
